Intense

Flat-premium ambulatory supplementary from enrollment age. Covers fitness, alternative medicine up to CHF 3,000/year, psychotherapy, glasses, and family planning. Ranked #1 supplementary by moneyland.ch 2024.

Flat from enrollment age — varies by canton

Health declaration required. Pre-existing conditions can lead to exclusions. The flat rate starts at your enrollment age, not universally at 30.

Mivita Reala

Mid-tier ambulatory, age-banded (5 age classes). Covers complementary medicine with tiered limits. For expats who want supplementary coverage without the Intense commitment.

Age-banded — lower than Intense at younger ages

Age-banded pricing means this product climbs as you age — unlike Intense. The coverage breadth is also narrower.

Comforta Semi-Private / Private

Hospital tiers. Semi-private: twin room, free doctor choice. Private: single room, chief physician. Flex option allows per-admission ward choice.

Age-banded — steepest of Atupri's products

Hospital tiers are age-banded even with Intense. The flat premium only applies to Intense ambulatory — hospital is separate.

Denta

Dental supplementary. Covers dental treatment contributions. Separate product from Intense — dental is not included in the flat-premium package.

Moderate — separate from Intense

Dental is a separate add-on. Some expats assume Intense covers everything — it doesn't cover dental. Add Denta explicitly.

Two services come with most plans: FlexCare (hybrid Telmed + HMO — available in Zürich, Bern, Lucerne) and Medgate tele-medicine (24/7 for Telmed models).

Atupri Intense: the premium you lock in at enrollment stays flat for life. The line doesn't climb. This is the only flat-premium ambulatory supplementary in Switzerland.

Flat from enrollment — not from 30.

Intense premiums climb through age classes until 30 (separate bands for 0–12, 13–25, 26–29). From 30 onward, the rate flattens — your enrollment-age premium is the premium you pay at 70.

A 30-year-old locks in the 30-year-old rate. A 45-year-old locks in the 45-year-old rate — higher, but still flat from that point forward. The flat curve is valuable at any enrollment age; it's most valuable the younger you start.

The catch: Intense requires a health declaration. If you wait until 55 to apply, the declaration is harder and the enrollment-age rate is high. The optimal window is 26–35. After that, Intense still works — but the advantage narrows.

Where Atupri frustrates you.

No English customer service. Phone and written support is German, French, and Italian only. The portal has a DE/EN translation tool — helpful for reading documents, not a substitute for resolving a disputed claim in your language.

Slow reimbursements. Multiple user reports cite 3–6 month waits for benefit payouts. Calling to follow up reportedly accelerates processing — but the default timeline is slower than CSS, Helsana, or Sanitas.

FlexCare is only in three cities. If you're in Lausanne, Geneva, Basel, or anywhere outside Zürich/Bern/Lucerne, the cheapest Atupri basic model isn't available. Standard and Telmed are national, but the pricing advantage narrows.

We placed him on Intense anyway. Even at the 53-year-old rate, the flat trajectory beats age-band competitors over the next fifteen years. The health declaration at 53 was the real risk — it came back clean.

Intense at 53 isn't cheap. But flat-expensive beats climbing-cheaper by year seven.

Late-career relocation. At 61, the Intense enrollment-age rate is steep — but it's the last rate increase he'll face. The age-80 clause means the flat rate holds for nearly two decades.

We compared Intense at 61 against CSS myFlex and Helsana COMPLETA age-band alternatives. Intense was more expensive today but cheaper by year five. The health declaration was the gate — it passed.

At 61, the question isn't whether flat is expensive. It's whether climbing is worse.

If the declaration is complicated, or if you need English-language service, or if you're outside the three FlexCare cities — we're looking at SWICA or Helsana instead.
